Note: The list of eligibility criteria and services may change when waiver amendments or waiver renewals are approved by the Centers for Medicare and Medicaid Services (CMS). Please, visit the Department of Public Welfare for a current list of all services provided through this waiver.
Be a resident of Pennsylvania
And be a US citizen or lawful permanent resident
Be between the ages of 18-59
Certain waivers have no age limit for individuals with an intellectual disability or autism
Be recommended for an Intermediate Care Facility (ICF) level of care
Based on a medical evaluation
Be eligible for Medical Assistance (MA)
Meet the financial requirements as determined by your local County Assistance office
You must earn $2,100 per month, or less
